I’m thinking of a scene where the main female character is on an island. She goes out swimming, gets a muscle cramp, and bites her tongue so hard it bleeds. The leg uncramps and she retuns to shore.

I thought it was Island of the Blue Dolphins. But no combination of search terms came up on Google.
