I am constantly being told how Fiddlesticks is scariest champion in the game. And I agree, visually and mimic wise designers absolutely nailed it. However in my option he is not scary at all outside of that, and that is simply due to his gameplay design not being terrifying. Which in a way matters even more than the visuals for the scary factor. Sure they gave him special passive for that, but they all depend on the jumpscare factor which is much less effective over exposure, not to mention them having some weird cycles. Like why would Fiddlesticks just walk out of the bush or flash straight onto me? That makes no sense, so its likely his lantern passive. However Warwick is entirely different story.

His design isn’t creepy at all, in fact a lot of modern champions very well outshine him in the visually scary factor. However the gameplay wise they had absolutely nailed it. He doesn’t scare you with cheap jumpscares but with your own paranoia. That deep prolonged soundtrack caused by his W as well as the screen getting filled with blood vapor really draws your mind to it. Causing you to get paranoid, knowing that he very well could be running at mach 5 right after you and you wouldn’t even be able to know. Its the feeling of him coming at any moment rushing down to finish you off. And the rest of the kits exists to compliment that. Towers are always seen as the save heaven. But not to Warwick, for he is one of the easiest divers in the game with his E damage reductions. Flashes and dashes are also pointless due to his Q. Your only hope of surviving him is trying to outplay him with stuns or recall (and the latter is very stressful knowing he can pop out at any moment). After reaching level 6, you might not even be able to anticipate him as he ults over the walls right to you.

Sure of course its just a game, and its not like the setting changes. But the visual and soundtrack effects alongside his gameplay supporting the instilling paranoia makes him in my opinion the scariest champion in the game.
