What do you consider the biggest letdowns, missed opportunities, falls from grace, bad decisions etc in the industries history?

My first thoughts (slightly obvious choices): Silent Hills cancellation
Big publishers pushing out talent and great franchises losing quality (Mass Effect and Dead Space?)
Great game franchises kept alive without the original talent, (silent hill? Halo?).
(I started thinking about this considering the potential future of arkane, and what redfall and Bethesda may mean for their future)
This a million times over. The micro transaction tied with live service gaming has turned most games into a cesspool of nickel and dining for things that should have an in game monetary system, not my cash.
