What do you consider the biggest letdowns, missed opportunities, falls from grace, bad decisions etc in the industries history?

My first thoughts (slightly obvious choices): Silent Hills cancellation
Big publishers pushing out talent and great franchises losing quality (Mass Effect and Dead Space?)
Great game franchises kept alive without the original talent, (silent hill? Halo?).
(I started thinking about this considering the potential future of arkane, and what redfall and Bethesda may mean for their future)
Stopping the Jak and Daxter series
The Blizzard and Activision merger screwed up so many Blizzard games. WoW got back to quality with Dragonflight but we’ll see how long that lasts
Ubisoft 😭 just ubisoft
For me downfall of Battlefield with BF 2042. Damn that EA ruined so many good franchises
Lack of Bloodborne 60 FPS PC port
The death of lionhead studios
Death of Gunpei Yokoi
Microsoft acquiring Rare, which led to the cancelation of Conker’s Bad Fur Day sequel.

Ubisoft existence
No Knights of the Old Republic 3.
Titanfall 2 being released in between battlefield and infinite warfare
We expect games to release at basically beta-testing stage and not get the 100% game until a patch or DLC two years later.
Pokemon games from 20 years ago being better and more refined than the Pokemon games released today.
Silent Hills is the one that hurt the most for me.
Thief remaster was a disaster, I grew on that series and they just went and butchered it.
