2010 mini cooper JCW skipped a tooth on the intake on a brand new chain

About 4000 miles ago I did my timing chain. Brand new OEM guides, chain, and tensioner. I’m in the process of doing my valve stem seals, and as I was setting the timing I noticed my intake cam was a tooth off. I verified every single thing was perfect when I did the job, so I know I did not mess this up before. Does anyone have any ideas as to why it would have skipped a tooth?
