Mine is when weapons lack color synergy with outfits/armor. I hate when for example, a character has a blue/silver color scheme while their sword is red/gold.

If theres a Silent Protagonist but all the dialogue options are the same
You’re not helping me “self-insert” more just cause the protagonist is silent. You’re actually taking me out of the experience more when all the options are approximately the same thing but maybe asking about something else.
If we’re talking visual design, then its basically every JRPG protagonist ever thats just your average anime protagonist. Normal nice guy. Regular life. Usually dragged into the plot either by random chance, because other characters drag them into it, or cause they help or are helped by pretty anime girl. I couldnt even play 13 Sentinels or Tales of Xilia cause of all the cliches I was seeing off the bat.
